Effectiveness of treatment with PEG-interferon and Ribavirin drugs

Description

Hepatitis C is an infectious disease leading to liver cirrhosis, liver failure, and hepatocellular carcinoma in patients with the chronic form. Hepatitis C virus (HCV) is subdivided into six genotypes. The most common form of the genotype - HCV-1 (genotype 1) accounts for about 75 percent of cases. Treatment of the chronic form of infection is usually carried out with a combination of PEG-interferon (PEG interferon alpha) and ribavirin (RBV) drugs, the effectiveness of which ranges from 40 to 50 percent.
The genotype of the interleukin 28B gene (lambda or type III interferons), for which the presence of two significant genetic markers (single nucleotide polymorphisms, SNPs) has been shown: rs12979860 C/T and rs8099917 T/G, located in the regulatory region of the gene, has a great influence on the effectiveness of HCV-1 treatment with PEG-interferon and ribavirin.
For SNP rs12979860, the CC genotype is associated with a two- to three-fold increase in the rate of sustained virological response (SVR) formation after therapy with PEG-interferon alpha and ribavirin, while the CT and TT genotypes are characterized by lower treatment response effectiveness.
For SNP rs8099917, the TT genotype is associated with a higher probability of SVR formation after therapy with PEG-interferon alpha and ribavirin, while the TG and GG genotypes respond less to treatment and hinder the achievement of SVR.

Most favorable genotype (high probability of achievement and rate of SVR formation)
rs12979860 CC, rs8099917 TT: 27.4% of the population
Highest SVR formation rate
rs12979860 CC, rs8099917 TG or GG: 10.4% of the population
Highest probability of achieving SVR
rs12979860 CT or TT, rs8099917 TT: 45.2% of the population
Least favorable genotypes (low probability of achievement and rate of SVR formation)
rs12979860 CT or TT, rs8099917 TG or GG: 17.0% of the population

Based on the results of the comprehensive study of significant genetic markers to identify the effectiveness of treatment with Ribavirin and PEG-interferon drugs, a geneticist's conclusion is issued, which will allow for more effective prescription of a treatment course and prognosis of its effectiveness.
