Description
It is a precursor of pepsin, synthesized by the chief cells of the gastric corpus glands. In small concentrations, pepsinogen I circulates in the blood and can normally be detected in urine. The level of secretion is controlled by gastrin and depends on the number of chief cells in the gastric corpus glands. Determination of pepsinogen I concentration is the gold standard in the diagnosis of atrophic gastritis of the gastric corpus, as the method has high sensitivity (up to 80%) and specificity (98%); together with determination of gastrin-17 level, it guarantees 100% specificity regarding the diagnosis of atrophic gastritis of the gastric corpus and antrum.
Increased levels are observed in: gastric ulcer, Zollinger-Ellison syndrome, acute gastritis, gastric cancer.
Decreased levels are noted in: atrophic gastritis of the gastric corpus, pernicious anemia.

2. Differential diagnosis of peptic ulcer disease and other diseases of the stomach and duodenum
3. Comprehensive diagnosis of malignant neoplasms of the stomach
4. Comprehensive diagnosis of pernicious anemia
